About the Provider Duties:
- Visits the patient at least twice weekly.
- Calls the responsible party/next of kin as well as referring provider shortly after admission to introduce themselves and to discuss the plan of care.
- Responds promptly (within 15 minutes for non-emergent calls) to the facility nurses.
- Promptly addresses any patient with an acute change in condition.
- Makes themse! lves available to the patient and/or family Physician or NP/PA! , when possible, assesses the patient within 4 hours of admission to the center members at all times.
- Treats patients and their family members with compassion at all times.
