St. Luke's University Health Network has a reserve Nurse Practitioner opportunity for its St. Luke's Warren Campus in Phillipsburg, NJ.
The Geriatric Nurse Practitio! ner participates in the assessment of the elderly patients in the home and works in conjunction with the geriatrician to perform physical examinations, functional evaluations, medication reviews etc. Provides client and caregiver teaching. Coordinates health screenings for people 55 and over. Assists in the development of geriatric education for the healthcare staff.

JOB DUTIES AND RESPONSIBILITIES:
Functions in the role of geriatric nurse practitioner for the Senior Assessment Service with duties to include home visits, physical exams, medication reviews and developing full report to family physician in conjunction with geriatrician and social worker
Coordinates health screenings/promotions for the 55 ! and over population
Provides geriatric education to h! ealthcare professionals
Oversees 2 osteoporosis exercise classes
Reviews findings of the Mindstreams Cognitve Assessmentsperformed in the department and develops recommendations to family physician
PHYSICAL AND SENSORY REQUIREMENTS:
Must be able to perceive attributes of an object through touch. Must be able to hear as it relates to normal conversation, and high and low frequencies, including using the telephone. Must be able to see as it relates to general, near, far, color, and peripheral vision. Must be able to push a wheelchair, if needed. Must be able to maneuver stairs in a client’s home.
EDUCATION
Masters of Science in Nursing; ANA Certified Gerontological Nurse Practitioner with 5 year renewal; current NJ license with biannual renewal
TRAINING AND EXPERIENCE:
Strong understanding and knowledge base in the aging process; ability to function in an interdisciplinary setting; strong knowledge ! of drug interactions as they relate to the older adult.
Job Qualifications:
Masters of Science in Nursing
ANA Certified Gerontological Nurse Practitioner with 5 year renewal
Current NJ license with biannual renewal
Knowledge of new trends in geriatrics
